Before submitting the form please note: 

  • Restart the phone and if possible, also remove the battery.
  • In case the phone doesn't start, charge the phone for at least 5 minutes and try again.
  • The new device can be delivered to a location where there is someone during office hours. We recommend the lobby or front desk of your company.
